Use: For taking more than 18 hours in semester
Directions: Students who wish to take more than 18 hours in a semester must use this form. Describe the reason or rationale for the request, sign and date it, print your advisor’s name in the gray block, and submit it to your advisor for approval and signature.
After receiving the advisor’s signature submit it to the registrar’s office. The registrar will verify your cumulative GPA and successful hours and submit it to the provost for a decision. The registrar’s office will notify you of the outcome.
