    Overload form

    Use: For taking more than 18 hours in semester

    Directions: Students who wish to take more than 18 hours in a semester must use this form. Describe the reason or rationale for the request, sign and date it, print your advisor’s name in the gray block, and submit it to your advisor for approval and signature.

    After receiving the advisor’s signature submit it to the registrar’s office. The registrar will verify your cumulative GPA and successful hours and submit it to the Provost for a decision. The registrar’s office will notify you of the outcome.

    Substitution/waiver form

    Use: Request for substitution or waiver of required credit

    Directions: Students who wish to make a change to the course requirements of their program must use this form. Complete the heading by entering name, ID# and the program in which you wish to make a substitution or waiver. In the gray box print the name of your advisor and the name of the department chair which oversees the program.

    List the course which is required but you do not want to take and check whether you want it waived or if you wish to take another course in its place. Describe your rationale. Sign and date.

    Submit to your advisor for his/her comments and approval. Submit it to the registrar. The registrar will make comments and recommend approval or disapproval then submit it to the department chair who will do the same.

    If the department chair and the registrar have different recommendations, the dean of the college in which the program resides will facilitate the resolution of the application.
