Physical Examination Record for Intercollegiate Sport Participation

Required for all athletes. Please note that all new students, athletes and non-athletes also fill out a basic medical history form for the university's health center.

Insurance & Authorization Form

Required for all athletes; also attach a copy of the front and back of your insurance card.

Parental Authorization

Required for athletes who are 18 years old or younger.
